If I drop a track into the sampler in-between 2 existing samples in 'list view' it will insert the sample by moving everything below it down a slot. If I then 'right-click>remove' the sample, it removes it but there is a gap left where is was, the samples below are not moved back up and there is no way I can see to rectify or re-order the samples so now I have gaps on my sampler pad I can't get rid of.
Existing banks do not have samples automatically created in their corresponding .oldbankfolder but adding any sample to an existing bank will create a .vdjsample file there for it, so some samples are there, some are not.
Add the sample again and you are asked if you want to overwrite the existing one, if you say 'yes' then the sample is 'moved' rather than duplicated, and all the samples re-order so there are no gaps again, however the slot numbers that appear on the left in list view do NOT update so are now totally wrong (eg. 54,55,51,57 etc).
I am struggling to create and edit sample banks under the current system as I don't know what it is going to do under any given circumstance.

Add a VIDEO sample to an existing sample bank, then load that sample into any deck.
(This will load the .vdjsample file from the .oldbank into the deck rather than the original video file)
Right click the sample and select 'Remove' (stupid I know but mistakes can happen and I am only removing the sample not trying to delete it).
However the software tries to recycle the item from the .oldbank folder since it's no longer needed but since it's loaded in a deck it can't and VDJ hangs and cannot be recovered.
This is stupidity on my part but in a live situation it is not the best outcome.

I also encountered your first issue recently, when trying to replace a sample (a still image) with a new one.
It seems you can't perform edits like that in the sideview (list) area. You have to select "edit bank in browser", which then means you lose the original contents of your browser while you're editing.
I think edits should be permitted in the sideview.

Take a video filmed on an iPhone in portrait mode rather than landscape.
Load it into a deck and it will play fine with black bars either side of it.
If you load it into a sample bank and play it, the video rotates 90deg and plays sideways.

Yes I realised that too, but once you've done it there is no going back, and you've been able to do it that way for as long as I can remember so I do it without thinking.
PS if you switch to 'pad' rather than 'list' view in sideview, you can drop samples to 'replace' what's in a pad slot, leaving the browser where it was.
Perhaps an option at the top to 're-order' samples so it removes any gaps and resets the slots would be an idea rather than just messing up the bank.

Being able to drag samples in is a new thing, as previously a dragged sample would load to a deck, but I'd expect to be able to order and/or replace slots and content without having to edit in the browser, or with things only working in one view but not the other.
